Subject:          NFHS Field Hockey Questionnaire for Officials and Coaches

The NFHS is distributing the fall sport-specific questionnaires. The questionnaires are accessed directly through the link below.  The state association needs only to provide the information via e-mail from your office to your respective field hockey officials and coaches for their participation in the questionnaire process or post the link on your state Web site.  The field hockey questionnaire will open on November 8 and remain open through November 30, 2012.

You will also receive today a second, separate e-mail with a different questionnaire link to be used only by you as the field hockey administrator to provide the state association response. If your state has more than one individual working with field hockey, please collaborate on your responses, as only one questionnaire per state will be accepted.

If you have any questions regarding the questionnaire process before you send the e-mail to your coaches and officials, please do not hesitate to contact Mike Meenan (mmeenan@nfhs.org) or Kathy Caudill (kcaudill@nfhs.org) of the NFHS staff.

Directions for Officials and Coaches Questionnaire Response

1.      State association sends out e-mail to the sport officials and coaches inviting them to participate in the NFHS Sport Season Questionnaire.  (There is no limit on the number of officials and coaches participating in the questionnaire process.)

·         The officials and coaches access the Questionnaire menu using the link below.

·         The individual then selects his/her appropriate sport to access and completes the questionnaire.

2.      Sample e-mail that could accompany the questionnaire link to your state’s officials and coaches:

You have been invited to take part in the NFHS rules-writing process by taking the annual field hockey rules questionnaire.  This questionnaire is provided so the opinions of a national sampling of coaches and registered officials will be considered by the NFHS Field Hockey Rules Committee.
